Integrating for Outcomes: Scaling Digital Solutions Across the Health and Health Care System

Overview

From connecting providers and patients, to enabling the collection and analysis of big data to inform programs, policy, and practice, digital health plays a key role in the reshaping our health and health care system.

Building on the successful fall 2017 meetings on Transforming Health Care Through Digital Innovations and the Strategic Foresight Workshop on How will eHealth Impact Canadian Health and Health Care in 2035, we invite you to join sector stakeholders and thought-leaders from The Conference Board’s Centre for the Future of Health (CFH) and The Canadian Alliance for Sustainable Health Care (CASHC), and invited guests, as they delve deeper into the subject of scale and spread of patient-oriented digital health solutions for integrated care and better health outcomes across the Canadian health care system.

To realize the full potential of digital health in an era of value-based health and health care, there is a need to build-on existing knowledge; learn from experiences; scale and spread best practices; actively engage patients, families, and caregivers; and support its optimization, such as through innovative funding models. Further, setting bold, aggressive goals with measurable targets and benchmarks is crucial to systemic digital health transformation—and this requires collaboration across the health ecosystem. Creating and supporting strategic partnerships across industry, government, provider, and patient networks can enable the development of innovative solutions and optimize the system-wide benefits of their scale and spread.

The overarching theme of the joint program is the enhancement of system integration for improved patient outcomes through the scale and spread of best practices and patient-oriented digital solutions, which will ultimately improve the sustainability of the health system.

The program will include dynamic presentations from the country’s top thought-leaders, interactive discussant panels, and a roundtable working session. Fittingly taking place at one of Canada’s largest innovation hubs, MaRS Discovery District in Toronto, this meeting is not to be missed!

Objectives

  • Learn about the shift to value-based care, and innovative approaches to leveraging digital technologies in Canada and abroad for improved health outcomes and better system integration;
  • Engage in discussion with thought-leaders on the opportunities to getting the most out of digital technologies in health and health care and the results we expect to see from optimal scale and spread;
  • Gain insights on approaches to, and the benefits of, engaging patients and caregivers in the design, implementation, and evaluation of innovation within the health care system.
