I am writing a review before I read this document. However, I will read it to see if there is hope in the " "access" to care" theme . I know that I will find the same " review" of that which "is". We are now in a time where radically different structures of care delivery are needed. The first step to permitting the entry of these new forms is : 1. creation of alternatives ( any reports/ arenas/ thinktanks/ blogs on this?) 2. dismantling of the old ( structured as a sanction) 3. with the provision of the new ( structured as an incentive) The incentive attractive to all?: circular independent flow of funds (KEY) gleaned from the tech coordination of core service activity . Now I will force myself to read what I already know I will find i.e. preservation of " the system"

| The health care system has been widely examined over many decades. Progress on changing/improving the delivery system has been slow, as has been the case elsewhere in the world. Perhaps the next research should be about the barriers to change and how to overcome them. Then, maybe we can move toward breaking down delivery silos and self-interests and achieving coordinated cost-effective care/service that gets better outcomes. |

| This is a useful background document that provides a helpful reminder of that there is life beyond Romanow and Kirby and the work done by provinces. And while it may be a bit self-serving, I am pleased that the survey includes the Fyke Report out of Saskatchewan. The Commission on Medicare was one of the highlights of my career! |

Timely and interesting report. I read it with a lot of professional interest as health system reform and health care system performance are one of my main themes of research.
I was however somewhat disappointed by the superficiality of the analysis and the lack of convincing conclusions about the pathways for action. |
